Usually they mention "without changing the meaning of the sentence". So as long as the meaning is same it can be direct or indirect (not very sure of this though). And you can say exclaimed. We actually say "Ravi exclaimed" because he is exclaiming. It it was an assertive sentence why would we say "Ravi exclaimed" at all?

ur answer to me appears corect...except the bug in the exclamatory sign...

You did not understand what I'm trying to say. Since Ravi is exclaiming in that particular sentence we say "Ravi exclaimed". But if Ravi was asking a question wouldn't you say "Ravi asked"? Or if it was an assertive sentence wouldn't you say "Ravi said"? So we can say "Ravi exclaimed".
